We are seeking a dedicated Registered Nurse with significant home health and OASIS experience for a 13-week, full-time contract. This role is ideal for professionals who are detail-oriented, excel at clinical documentation, and are ready to support patients at a pivotal time in their lives.
Schedule:
- 13-week assignment with potential to extend
- Full-time, Monday through Friday
- On-call responsibilities may be included
- Structured daily visit expectations
Key Responsibilities:
- Deliver in-home nursing care and skilled clinical interventions
- Conduct patient assessments and complete timely, accurate OASIS documentation
- Collaborate with physicians and interdisciplinary care teams to create individualized care plans
- Provide wound care, manage medications, offer disease education, and monitor patient progress
- Clearly communicate changes in patient conditions to the clinical team
Qualifications:
- Active RN license
- At least one year of recent experience in home health RN roles
- Proficiency with OASIS documentation is required
- Experience with HomeCare HomeBase software is strongly preferred
- Able to maintain a structured, full-time schedule with multiple patient visits each day
- Reliable transportation and a valid drivers license
