An experienced Registered Nurse (RN) with a background in home health is needed for a contract role in Shepherdsville, KY. This opportunity focuses on delivering comprehensive nursing care in patients homes, emphasizing strong clinical skills and proficient use of OASIS documentation.
Key Responsibilities:
- Conduct initial and ongoing patient assessments, including vital signs, medication management, and wound care.
- Develop and implement personalized care plans tailored to patient needs.
- Educate patients and their families about disease management, treatment plans, and preventive measures.
- Collaborate effectively with physicians, therapists, and other healthcare professionals to coordinate care.
- Accurately document patient care activities in electronic medical records (EMR).
- Monitor patient condition changes and report to supervising physicians.
- Ensure adherence to state and federal home health care regulations.
Qualifications:
- Active RN license.
- Minimum of 3 months experience in home health nursing utilizing OASIS.
- Experience with HCHB is highly desirable.
- Strong clinical skills, including IV therapy, wound care, and medication management.
- Proficiency in electronic medical records (EMR) systems.
- Excellent communication and interpersonal abilities.
- Ability to work independently and manage time effectively.
- Valid drivers license and reliable transportation.
