Your next nursing opportunity starts here. We are seeking an experienced Registered Nurse with a strong background in home health and OASIS documentation for a 13-week full-time contract. This role is perfect for a clinically confident RN who excels in structured scheduling, thorough documentation, and patient-centered care in the home setting.
Schedule:
- 13-week contract, potential to extend
- Full-time, Monday through Friday
- On-call responsibilities may be included
- Daily structured patient visit requirements
Key Responsibilities:
- Deliver skilled nursing care and interventions in patients homes
- Complete timely and accurate OASIS assessments and documentation
- Collaborate closely with physicians and interdisciplinary care teams to develop and implement individualized care plans
- Provide wound care, medication administration, patient education, and monitor for changes in patient condition
- Communicate effectively with the clinical team regarding patient progress and any changes in status
Preferred Qualifications:
- Active Registered Nurse license
- At least 1 year of recent experience in home health nursing
- Required proficiency with OASIS documentation
- Experience with HomeCare HomeBase is strongly preferred
- Strong organizational skills and the ability to manage a set, full-time schedule
- Reliable transportation and a valid drivers license
