We are seeking a dedicated Registered Nurse (RN) with proven OASIS experience for a full-time, 13-week home health contract assignment. If you excel at clinical documentation, enjoy creating meaningful patient relationships, and thrive on managing a set schedule, this role offers the chance to truly make a difference.
Schedule:
- 13-week contract, Monday through Friday
- Full-time hours with potential on-call requirements
- Structured daily visit assignments
Key Responsibilities:
- Deliver skilled in-home nursing care for clients
- Complete comprehensive patient assessments, ensuring accurate and timely OASIS documentation
- Collaborate with physicians and interdisciplinary care teams to develop individualized patient care plans
- Perform wound care, medication administration, patient and family education, and disease management
- Promptly communicate changes in patient status to the wider clinical team
Required Qualifications:
- Active, unencumbered RN license
- At least 1 year of recent home health RN experience
- Strong proficiency with OASIS documentation
- Experience with HomeCare HomeBase is strongly preferred
- Ability to manage a full, structured daily visit schedule
- Reliable transportation and valid drivers license
