A rewarding 13-week full-time assignment is available for an experienced Registered Nurse specializing in home health care. This contract role is perfect for a clinical professional with strong OASIS documentation abilities, excellent assessment skills, and a commitment to delivering high-quality care in a structured environment.
Schedule:
- Full-time, Monday through Friday
- On-call duties are probable
- Structured daily patient visit expectations
Key Responsibilities:
- Deliver comprehensive in-home nursing services and skilled interventions
- Perform patient assessments, ensuring OASIS documentation is complete and submitted on time
- Collaborate with physicians and care team members to create and execute individualized care plans
- Oversee wound care, medication administration, disease monitoring, and patient/family education
- Maintain timely communication with the clinical team regarding patient updates and changes in condition
Qualifications:
- Active RN license
- At least one year of recent RN home health experience
- OASIS documentation experience is required
- Proficiency with HomeCare HomeBase is strongly preferred
- Ability to maintain a structured, full-time schedule involving daily patient visits
- Reliable vehicle and valid drivers license
