Your next nursing opportunity starts here. We seek a dedicated Registered Nurse experienced in home health to fill a full-time, 13-week contract. Schedule:

• Full-time, Monday through Friday
• On-call shifts likely
• Daily home visit structure

Key Responsibilities:

• Deliver skilled in-home nursing interventions with compassion and accuracy.
• Complete patient assessments and OASIS documentation thoroughly and punctually.
• Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
• Perform wound care, medication administration, patient education, and monitor disease progression.
• Communicate ongoing patient status and significant clinical changes to the care team.

Preferred Qualifications:

• Current, active RN license.
• At least 1 year of recent experience as a home health RN.
• OASIS documentation expertise required.
• High proficiency with HomeCare HomeBase (strongly preferred).
• Dependable transportation and a valid drivers license.
• Self-motivated with excellent time management and organizational skills.
