A leading healthcare provider is seeking a dedicated Registered Nurse (RN) with expertise in home health and experience with OASIS for a 13-week full-time contract. This position offers the chance to deliver vital care to patients in their homes while collaborating closely with a professional, supportive care team to ensure optimal patient outcomes.
Schedule and Hours:
- Full-time contract (13 weeks), with potential extension
- Monday through Friday schedule; on-call duties may also be assigned
- Daily visit targets and structured expectations
Key Responsibilities:
- Deliver in-home patient care including skilled nursing interventions
- Complete accurate, timely OASIS documentation and patient assessments
- Develop and implement individualized care plans in partnership with physicians and interdisciplinary teams
- Carry out wound care, medication administration, disease process monitoring, and patient/family education
- Maintain frequent communication with the clinical team to report changes in patient conditions
Desired Qualifications:
- Active and unrestricted RN license
- Minimum 1 year of recent home health RN experience
- Proficient in OASIS documentation (required)
- Experience with HomeCare HomeBase software (strongly preferred)
- Reliable personal transportation and valid drivers license
- Ability to independently manage a full-time schedule and daily patient visits
