Your next nursing opportunity starts here. An experienced Registered Nurse (RN) with a strong background in Home Health is needed for a 13-week full-time contract. Schedule:

• Full-time, Monday through Friday
• On-call responsibilities may be required
• 13-week contract, with the possibility of extension

Key Responsibilities:

• Deliver skilled, compassionate nursing interventions in the home setting
• Perform comprehensive patient assessments and complete OASIS documentation accurately and promptly
• Collaborate with physicians and interdisciplinary teams to ensure personalized care plans
• Provide wound care, medication administration, disease monitoring, and patient/family education
• Report changes in patient condition and ensure proper clinical communication

Qualifications:

• Active RN license in good standing
• Minimum of 1 year recent RN Home Health experience
• Required OASIS documentation expertise
• Proficiency with HomeCare HomeBase is highly preferred
• Ability to manage a structured schedule and daily patient visits
• Reliable transportation and valid drivers license
