This full-time contract is looking for a dedicated Registered Nurse (RN) experienced in OASIS for a 13-week assignment. Showcase your skills in patient assessment, documentation accuracy, and home health care delivery as part of a professional, collaborative team.
Schedule:
- 13-week contract with potential for extension
- Full-time, Monday through Friday
- Daily structured patient visit expectations
- On-call responsibilities may be required
Key Responsibilities:
- Deliver skilled, compassionate nursing care to patients in a home setting
- Perform comprehensive assessments, and document accurately and efficiently using OASIS
- Collaborate with physicians and interdisciplinary care teams to customize patient care plans
- Administer wound care, medication management, patient/family education, and disease monitoring
- Proactively communicate changes in patient condition to the clinical team
Qualifications:
- Active RN license
- Minimum of one year of recent home health RN experience
- Prior OASIS documentation experience is required
- Proficiency with HomeCare HomeBase strongly preferred
- Reliable transportation and a valid drivers license
- Ability to manage a structured schedule and a caseload of daily visits
