We're seeking a skilled Registered Nurse (RN) with strong home health experience and OASIS competency for a 13-week full-time contract. This role is ideal for nurses who excel in managing daily patient visits, maintaining thorough documentation, and delivering individualized, compassionate care in a home setting.
Schedule:
- 13-week contract, with potential to extend
- Full-time hours: Monday through Friday
- On-call duties may be required
Key Responsibilities:
- Deliver in-home nursing care, including patient assessments and skilled interventions
- Accurately complete OASIS documentation and submit within required timelines
- Collaborate with physicians and multidisciplinary care teams to create and implement patient-specific plans
- Manage wound care, medication administration, disease monitoring, and provide thorough patient/family education
- Ensure timely communication of any changes in patient status to the clinical team
Who You Are:
- Active, unencumbered RN license
- Minimum one year of recent home health RN experience
- Demonstrated OASIS documentation proficiency (required)
- Experience with HomeCare HomeBase (highly preferred)
- Reliable transportation and valid drivers license
- Ability to work a structured, full-time schedule and efficiently manage daily visit expectations
