An exciting opportunity is available for a skilled and compassionate Registered Nurse (RN) with expertise in Home Health care to support patients in a rewarding contract role. This position is ideal for someone who thrives on making a difference in the home setting and is looking to leverage their experience with OASIS documentation in a collaborative environment.
Schedule/Hours:
- Contract position with flexible scheduling options depending on case assignments.
Qualifications:
- Active RN license
- At least 3 months of home health experience with use of OASIS documentation
- HCHB (HomeCare HomeBase) experience is highly desirable
- Strong clinical abilities: medication management, wound care, IV therapy
- Confident in EMR (Electronic Medical Records) documentation
- Excellent communication skills, both written and verbal
- Demonstrated ability to work autonomously and manage time effectively
- Valid drivers license and dependable transportation
Key Responsibilities:
- Deliver comprehensive, compassionate nursing care to patients in their homes
- Complete initial and ongoing patient assessments, including vital signs, wound care, and medication administration
- Develop and implement tailored care plans to meet each patients healthcare needs
- Provide education to patients and families on managing health conditions and treatments
- Collaborate closely with physicians, therapists, and healthcare team members for coordinated patient care
- Accurately record interventions, updates, and outcomes in EMR in a timely manner
- Monitor patient conditions and promptly report changes to the supervising provider
- Ensure all care complies with applicable state and federal home health regulations
