In this full-time, 13-week contract position, you will work within a supportive healthcare setting, ensuring individuals receive personalized attention and expert clinical support.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Perform thorough OASIS assessments and develop individualized care plans.
• Administer skilled nursing interventions, including wound care, IV therapy, medication management, and chronic condition support.
• Monitor patient conditions, evaluate progress, and communicate critical updates to the clinical care team and physicians.
• Educate patients and their families on diagnoses, care protocols, and medications to foster engagement in health management.
• Accurately and timely document all care activities using Home Care Home Base (HCHB).
• Coordinate closely with physical and occupational therapists, social workers, and home health aides for seamless care integration.
• Take part in case conferences, continuing education, and staff meetings to maintain best practices.
• Uphold all regulatory guidelines and organizational standards for in-home nursing care.

Qualifications:

• Current RN license in good standing for the state of practice.
• At least 1 year of recent home health experience; field experience strongly valued.
• Proficient in Home Care Home Base (HCHB) and experienced with OASIS assessments.
• Reliable vehicle, valid drivers license, and up-to-date auto insurance for daily travel.

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
