Experienced Registered Nurses are invited to join a dedicated home health team on a full-time, 13-week contract, providing vital care throughout the patient community. This is a field-based position with a MondayFriday, 8:00 AM 5:00 PM schedule, focusing on compassionate, skilled nursing within patient homes. Expect a manageable caseload with an average of 35 patient visits per day, utilizing your expertise with the Home Care Home Base (HCHB) system and OASIS assessment tool.

Schedule & Setting:

• Monday Friday, 8:00 AM 5:00 PM
• Caseload: 30 points per week (about 35 visits each day)
• Field-basedtraveling to patient homes within a defined territory
• Electronic documentation with HCHB; OASIS assessments

Qualifications:

• Active, unrestricted RN license in the state
• Minimum 1 year of recent home health nursing experience
• Proficiency in Home Care Home Base (HCHB) and OASIS required
• Reliable vehicle, valid drivers license, and auto insurance

Primary Responsibilities:

• Perform comprehensive OASIS assessments and develop individualized care plans
• Deliver skilled nursing care, including wound care, IV therapy, medication administration, and chronic disease management
• Monitor patient progress, communicate changes promptly to the interdisciplinary care team and providers
• Educate patients and families on treatment, medications, and preventive strategies
• Maintain thorough and timely electronic documentation
• Collaborate closely with therapists, medical social workers, and home health aides to coordinate care
• Participate in staff meetings, case conferences, and professional development
• Comply with all regulatory guidelines and agency protocols

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
