Job Description

Deliver high-quality nursing care to patients in their own homes as a key member of a dedicated home health team. This full-time, 13-week contract offers a rewarding field-based role, providing skilled nursing services and holistic care coordination in a supportive environment.

Schedule & Setting:

• Monday through Friday, 8:00 AM 5:00 PM
• Field-based: daily travel to patient homes within an assigned territory (mileage reimbursement included)
• Caseload expectation of 30 points per week, typically 35 patient visits per day

Qualifications:

• Active RN license in the state of practice (in good standing)
• At least 1 year of recent Home Health nursing experience (field highly preferred)
• Solid experience with OASIS and proficiency in Home Care Home Base (HCHB)
• Reliable transportation, valid drivers license, and current auto insurance

Primary Responsibilities:

• Conduct thorough OASIS assessments and develop individualized care plans for each patient
• Deliver skilled nursing interventions including wound care, IV therapy, medication administration, and chronic disease management
• Monitor patient progress, communicate changes in condition, and coordinate timely care with interdisciplinary team members and physicians
• Provide education to patients and families regarding disease processes, medications, plan of care, and preventative strategies
• Maintain timely and comprehensive electronic documentation using Home Care Home Base (HCHB)
• Collaborate with therapy staff, medical social workers, and aides to ensure seamless, coordinated care delivery
• Participate in regular case conferences, staff meetings, and ongoing professional development
• Uphold all regulatory standards for documentation and patient care

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
