Deliver high-quality nursing care to patients in their homes as part of a dedicated home health team in Seattle, WA. This full-time, 13-week contract is ideal for Registered Nurses with strong home health field experience and proficiency in OASIS and Home Care Home Base (HCHB) documentation.

Schedule & Hours:

• Monday to Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans
• Provide skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management
• Monitor patient progress, assess changes in condition, and communicate with the care team and physicians
• Educate patients and families on disease management, treatment plans, and medications
• Maintain accurate and timely electronic documentation in HCHB
• Collaborate with an interdisciplinary team including therapists, social workers, and aides
• Participate in case conferences, staff meetings, and continuing education opportunities
• Ensure adherence to all regulatory and agency standards for documentation and patient care
• Daily travel to patient homes within assigned territory (mileage reimbursement provided)

Qualifications:

• Active RN license in Washington (in good standing)
• Minimum 1 year recent home health nursing experience (field-based experience highly preferred)
• Proficiency in Home Care Home Base (HCHB) and required OASIS expertise
• Reliable transportation, valid drivers license, and current auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
