Schedule:
13-week contract, with potential for extension
Full-time hours, Monday through Friday
On-call rotations may be required
Structured daily visit schedule

Key Responsibilities:
Deliver safe, effective in-home nursing care and skilled clinical interventions
Conduct thorough patient assessments, ensuring accurate and timely OASIS documentation
Collaborate with physicians and interdisciplinary teams to coordinate individualized care plans
Provide wound care, medication administration, patient/family education, and ongoing health monitoring
Communicate patient status updates and any significant changes to the clinical team

Desired Qualifications:
Current, active Registered Nurse license
At least 1 year of recent experience as a home health RN
Demonstrated OASIS documentation experience (required)
Proficiency with HomeCare HomeBase is strongly preferred
Comfortable with structured, full-time schedules and meeting daily visit expectations
Reliable transportation and valid drivers license
