Schedule:

• 13-week contract, full-time
• Monday through Friday
• On-call duties may be required

Responsibilities:

• Deliver in-home nursing interventions and skilled care to patients
• Complete thorough assessments and OASIS documentation promptly and accurately
• Coordinate care with physicians and other professionals to develop and execute individualized treatment plans
• Perform wound care, medication administration, patient education, and disease surveillance
• Communicate patient status changes effectively to the care team

Qualifications:

• Valid RN license
• At least 1 year of recent experience in home health as a Registered Nurse
• Demonstrated expertise with OASIS documentation (required)
• Familiarity with HomeCare HomeBase (strongly preferred)
• Reliable vehicle and valid drivers license
• Ability to manage daily patient caseload and schedule efficiently

$2,193 - $2,312/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
