This position is ideal for an RN with OASIS experience who thrives in a schedule-driven environment and wants to make a meaningful difference in patients lives.
Schedule:
- Full-time, Monday through Friday
- On-call commitment may be required
- Structured daily visit expectations
Key Responsibilities:
- Deliver in-home nursing care and skilled interventions to diverse patient populations
- Accurately complete OASIS documentation and timely patient assessments
- Develop and implement individualized care plans in collaboration with physicians and interdisciplinary teams
- Provide wound care, medication management, disease monitoring, and education to patients and families
- Communicate patient status, updates, and care needs effectively to the clinical team
Qualifications:
- Active RN license in good standing
- At least one year of recent home health RN experience
- Demonstrated proficiency with OASIS documentation (required)
- Experience with HomeCare HomeBase is strongly preferred
- Reliable transportation and a valid drivers license
- Strong organizational skills and ability to confidently manage a full caseload
