A 13-week full-time assignment awaits a motivated Registered Nurse (RN) with strong OASIS skills, ready to impact patient lives in a home health setting. This contract role is ideal for professionals confident in clinical judgment, accurate documentation, and independent schedule management.
Schedule:
- Full-time, Monday through Friday
- On-call rotation may be included
- Structured daily visit targets
Key Responsibilities:
- Deliver patient-centered nursing care and skilled clinical interventions in home settings
- Conduct comprehensive assessments and complete OASIS documentation promptly and accurately
- Develop and execute individualized care plans in collaboration with physicians and interdisciplinary teams
- Perform wound care, medication administration, patient teaching, and chronic disease monitoring
- Maintain proactive communication with the care team regarding patient status and updates
Required Qualifications:
- Active RN license
- At least one year of recent experience as a home health RN
- Demonstrated expertise with OASIS documentation
- Proficiency with HomeCare HomeBase (highly preferred)
- Committed to fulfilling a steady, structured daily visit schedule
- Possession of reliable transportation and a valid drivers license
