Your next nursing opportunity starts here. We are seeking an experienced Registered Nurse with a strong background in home health and OASIS documentation for a 13-week full-time contract. This important role offers the chance to make a meaningful difference in patients lives while working within a supportive and collaborative care environment.
Schedule:
- Full-time, Monday through Friday
- On-call duties are likely
- Structured expectations for daily home visits
Key Responsibilities:
- Deliver attentive in-home nursing care and skilled interventions to patients
- Perform comprehensive assessments and accurately complete OASIS documentation within established timelines
- Develop and implement individualized care plans in collaboration with physicians and interdisciplinary teams
- Provide wound care, medication management, patient and caregiver education, and disease management
- Maintain timely communication regarding patient condition changes to the clinical team
Qualifications:
- Active RN license in good standing
- Minimum of 1 year recent experience in home health as an RN
- Demonstrated expertise with OASIS assessments and documentation
- Proficiency with HomeCare HomeBase is strongly preferred
- Ability to efficiently manage a structured schedule with multiple daily patient visits
- Reliable transportation and a valid drivers license
