We are seeking a dedicated Registered Nurse (RN) with proven home health experience and OASIS documentation skills for a full-time, 13-week contract assignment. Confidence in clinical care, accuracy in documentation, and the ability to manage a set schedule will empower you to make an immediate difference in patients lives while working within a collaborative and professional care environment.
Schedule:
- 13-week contract with the possibility of extension
- Full-time, Monday through Friday
- On-call availability may be required
Key Responsibilities:
- Deliver skilled, compassionate in-home nursing care, including patient assessments, wound care, medication management, disease monitoring, and education
- Accurately complete OASIS documentation and submit timely reports
- Collaborate effectively with physicians and interdisciplinary care teams to develop and manage individualized care plans
- Maintain regular communication about changes in patient condition
- Adhere to assigned daily visit expectations and prioritize quality care
Qualifications:
- Active RN license
- At least one year of recent RN home health experience
- OASIS documentation proficiency required
- Strongly prefer experience with HomeCare HomeBase EMR
- Strict adherence to a structured, full-time schedule requiring daily patient visits
- Reliable vehicle and current drivers license
