Step into a rewarding field-based nursing role, providing essential care to patients in their homes across Saint Petersburg, FL. This full-time, 13-week engagement offers a dynamic environment for skilled nurses passionate about home health, working Monday through Friday from 8:00 AM to 5:00 PM. Expect a manageable caseload, typically 35 patient visits daily, with a weekly goal of 30 points.

Key Qualifications:

• Current and active RN license in good standing
• At least 1 year of recent experience in home health (field work highly valued)
• Proficient in Home Care Home Base (HCHB) electronic documentation
• OASIS assessment tool experience is essential
• Reliable transportation, valid drivers license, and active auto insurance

Primary Responsibilities:

• Conduct comprehensive OASIS assessments to develop and implement individualized care plans
• Deliver skilled nursing care, including wound management, medication administration, IV therapy, and chronic disease monitoring
• Monitor and assess patient progress, promptly flagging and communicating status changes
• Educate patients and families about care plans, treatment options, and preventive measures
• Maintain accurate, up-to-date documentation within the Home Care Home Base system
• Collaborate closely with interdisciplinary team memberstherapists, medical social workers, and home health aidesfor coordinated patient care
• Join regular case conferences, staff meetings, and optional continuing education opportunities
• Uphold all agency and regulatory standards for high-quality patient care
