A rewarding opportunity awaits an experienced RN to deliver skilled care to patients in their own homes. This 13-week, full-time contract follows a Monday through Friday schedule, 8:00 AM to 5:00 PM, focusing on personalized home health visits across an assigned territory. With an expectation of 35 patient visits per day (based on a 30-point caseload per week), youll enjoy the autonomy of field-based work while playing a vital role in patient healing and independence.

Key responsibilities include:

• Conduct comprehensive OASIS assessments to develop individualized care plans
• Administer skilled nursing care such as wound care, medication administration, IV therapy, and management of chronic conditions
• Assess patient progress, recognize changes in condition, and communicate effectively with the care team and physicians
• Educate patients and families regarding disease processes, treatment plans, and preventive care
• Ensure accurate documentation using Home Care Home Base (HCHB)
• Collaborate with interdisciplinary team memberstherapists, social workers, and aidesto coordinate care
• Participate in case conferences, staff meetings, and ongoing professional development
• Uphold regulatory requirements and industry standards in documentation and care delivery

To excel in this position, you will need:

• Current, active RN license (state of practice) in good standing
• At least one year of recent experience in Home Health (field experience required)
• Proficiency in Home Care Home Base (HCHB) and OASIS assessment tools
• A reliable vehicle, valid drivers license, and current auto insurance

$1,113 - $1,178/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
