Begin a rewarding role providing essential home health nursing care in a field-based position serving patients in the Saint Paul, MN area. This full-time, 13-week contract lets you make a real difference by delivering skilled care, comprehensive assessments, and compassionate support directly in patients homes.

Schedule & Setting:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based visits within assigned territory (mileage reimbursement provided)
• Manage an average caseload of 30 points per week (35 patient visits daily)

Key Responsibilities:

• Complete comprehensive OASIS assessments and develop tailored care plans
• Deliver skilled nursing interventions including wound care, IV therapy, medication administration, and chronic disease management
• Monitor patient progress, promptly recognize condition changes, and communicate effectively with the care team and physicians
• Educate patients and families on disease management, medication administration, and preventive health
• Maintain accurate, timely documentation in the Home Care Home Base (HCHB) electronic system
• Collaborate with interdisciplinary staffincluding therapists, medical social workers, and home health aidesto coordinate and optimize care
• Participate in case conferences, staff meetings, and ongoing professional development opportunities
• Ensure compliance with regulatory and agency standards for all care and documentation

Qualifications:

• Active RN license in good standing in Minnesota
• At least one year of recent Home Health nursing experience (field experience strongly preferred)
• Proficient with Home Care Home Base (HCHB) software and OASIS data collection
• Reliable transportation, active drivers license, and current auto insurance

$1,113 - $1,178/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
