Make a direct impact on patient health and independence by providing expert RN-level care in a home health setting. This full-time, 13-week assignment offers a structured schedule, an opportunity to support patients within their homes, and a collaborative, multidisciplinary approach.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop tailored care plans for each patient.
• Deliver skilled nursing interventions such as wound care, IV therapy, medication administration, and chronic disease monitoring.
• Monitor and evaluate patients conditions, promptly sharing changes or concerns with the clinical team and physicians.
• Educate patients and their families on disease management, medications, and ongoing care requirements.
• Complete all documentation accurately and efficiently using Home Care Home Base (HCHB).
• Partner with physical and occupational therapists, social workers, and home health aides to ensure integrated care plans.
• Regularly participate in staff meetings, interdisciplinary case conferences, and ongoing training.
• Follow regulatory and organizational guidelines for documentation and delivery of care.
• Daily travel within the assigned territory; mileage reimbursement is provided.

Qualifications:

• Current, active RN license in good standing for the state of practice
• At least one year of recent, hands-on home health nursing experience (field experience preferred)
• Proficiency in Home Care Home Base (HCHB) and prior OASIS assessment experience
• Dependable vehicle, valid drivers license, and current auto insurance

$1,113 - $1,178/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
