Provide essential nursing care in a home health setting, supporting individuals in their transition from hospital to home while promoting patient independence and wellness.

Schedule/Hours:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Field-based (patient homes)
• Caseload: 30 points per week (typically 35 patient visits daily)

Key Qualifications:

• Active RN License in the state of Minnesota (in good standing)
• Minimum 1 year of recent Home Health experience (field experience preferred)
• Proficiency with Home Care Home Base (HCHB)
• OASIS assessment experience required
• Reliable transportation, valid drivers license, and current auto insurance

Primary Responsibilities:

• Complete thorough OASIS assessments and develop patient-specific care plans
• Deliver skilled nursing care including wound care, IV therapies, medication administration, and chronic disease management
• Closely monitor patient progress, promptly reporting clinical changes to the multidisciplinary care team and physicians
• Educate patients and caregivers on medication management, disease processes, and preventive health
• Document all care accurately and in a timely manner using HCHB
• Collaborate with home health professionals and participate in ongoing staff meetings, case conferences, and continuing education
• Uphold all regulatory, compliance, and agency-specific standards in care delivery

$1,113 - $1,178/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
