Schedule & Work Details:

• Monday through Friday, 8:00 AM 5:00 PM
• Field-baseddaily travel to patient homes within your designated territory
• Expectation of 30 points per week, averaging 35 patient visits each day

Qualifications:

• Active and unencumbered RN license in state of practice
• At least 1 year of recent home health experience, with fieldwork strongly preferred
• Proficiency using Home Care Home Base (HCHB) for electronic documentation
• Experience performing OASIS assessments required
• Dependable transportation, valid drivers license, and up-to-date auto insurance

Key Responsibilities:

• Complete thorough OASIS patient assessments and create personalized care plans
• Deliver skilled nursing interventions: wound care, IV therapy, medication administration, and management of chronic conditions
• Track patient progress, identify condition changes, and promptly communicate updates to clinical teams and physicians
• Offer education and support to patients and families regarding health conditions, medications, and preventive care
• Document all care accurately and in a timely manner using HCHB
• Collaborate closely with physical/occupational therapists, social workers, and home health aides to achieve optimal patient outcomes
• Attend case conferences, staff meetings, and professional development activities as necessary
• Uphold regulatory and agency standards for patient care and documentation

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
