Take your skills on the road with a field-based nursing position focused on delivering high-quality care to patients in their homes. This full-time contract role spans 13 weeks, offering daily variety, professional challenge, and a meaningful impact on patient well-being.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct thorough OASIS assessments and develop tailored care plans.
• Deliver skilled nursing interventions including wound care, IV therapy, medication administration, and chronic disease management.
• Monitor each patients progress, promptly report changes, and coordinate care planning with physicians and the interdisciplinary team.
• Educate patients and families on treatment regimens, medications, and proactive health practices.
• Complete timely and accurate documentation using Home Care Home Base (HCHB).
• Collaborate with therapists, medical social workers, and home health aides to provide cohesive, patient-centered care.
• Actively participate in case conferences, staff meetings, and ongoing professional development.
• Comply with regulatory guidelines and organizational policies for documentation and quality care delivery.
• Travel daily to patient homes within your assigned area (mileage reimbursement is provided).

Qualifications:

• Current RN license in good standing for the state of practice.
• At least one year of recent home health experience with fieldwork preferred.
• Proficiency in Home Care Home Base (HCHB) and strong OASIS assessment experience are required.
• Valid drivers license, reliable transportation, and active auto insurance.

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
