Step into a 13-week full-time assignment providing essential clinical care and supporting recovery in a home setting.
Schedule:
- Full-time, Monday through Friday
- Structured daily visit expectations
- On-call coverage may be required
Key Qualifications:
- Current, active RN license
- At least one year of recent experience as a home health RN
- OASIS documentation proficiency required
- HomeCare HomeBase experience strongly preferred
- Reliable transportation and valid drivers license
- Ability to manage a set schedule with daily patient visits
Core Responsibilities:
- Deliver skilled in-home nursing care and interventions according to individualized care plans
- Perform thorough patient assessments and complete timely, accurate OASIS documentation
- Collaborate closely with physicians and interdisciplinary teams
- Provide wound care, medication management, and disease monitoring
- Educate patients and families for optimal self-care at home
- Communicate updates and patient status changes to clinical leadership
