As an experienced Registered Nurse with strong OASIS skills, youll provide essential care, accurate documentation, and compassionate support to individuals in their own homes. This meaningful role offers the chance to make a daily impact while working alongside a collaborative, professional care team.
Schedule:
- 13-week assignment, with potential for extension
- Full-time, Monday through Friday
- On-call duties likely
Qualifications:
- Active RN license
- At least 1 year of recent home health RN experience
- OASIS assessment and documentation proficiency
- Familiarity with HomeCare HomeBase (highly preferred)
- Reliable vehicle and valid driver's license
What Youll Do:
- Deliver skilled, in-home nursing care and interventions to diverse patients
- Complete comprehensive assessments and timely, accurate OASIS documentation
- Coordinate closely with physicians, caregivers, and the broader health team to implement tailored care plans
- Manage wound care, medication administration, chronic disease monitoring, and patient education
- Clearly communicate updates on patient status to the clinical team
