In this role, youll directly coordinate, deliver, and supervise home health nursing care in accordance with physician orders and accepted agency procedures. Schedule/Hours:
- Home-based visits in the community
- Periodic office meetings required
Qualifications:
- Active, unencumbered Registered Nurse (RN) license valid in assigned state (NLC multi-state licensure preferred if applicable)
- Graduation from an accredited nursing program (BSN preferred in some locations)
- Minimum of 2-3 years of recent nursing experience
- Current CPR certification
- Proven ability to assess and care for patients with varying needs
- Reliable transportation, a valid drivers license, and agency-required insurance
- Computer literacy for electronic documentation and training
Key Responsibilities:
- Perform patient assessments and develop individualized care plans
- Deliver skilled nursing care and treatments in patient homes
- Direct, supervise, and evaluate Home Health Aides and LPNs/LVNs
- Document clinical findings, communicate changes to physicians, and coordinate care within the healthcare team
- Provide patient and family education, preventive care, and counseling
- Ensure compliance with state, federal, and agency regulations
