Step into an impactful role delivering high-quality care for patients in their homes. This full-time, 13-week contract position is ideal for registered nurses passionate about patient-centered care and skilled in the home health setting.

Schedule & Setting:

• Monday through Friday, 8:00 AM 5:00 PM
• Field-based, providing care in patient homes
• Expect 30 points per week, translating to approximately 35 patient visits each day
• Daily travel within an assigned territory (mileage reimbursement provided)

Qualifications:

• Active RN license in good standing for the state of practice
• At least 1 year of recent home health nursing experience preferred
• Proficient with Home Care Home Base (HCHB) electronic documentation
• Familiarity and experience with OASIS assessment tools
• Valid drivers license, reliable transportation, and current auto insurance

Responsibilities:

• Conduct thorough OASIS assessments and craft personalized care plans
• Deliver expert nursing care such as wound care, IV therapy, medication administration, and chronic disease management
• Monitor patient progress, recognize changes in condition, and communicate effectively with the care team and physicians
• Provide patient and family education on disease management, medications, and preventative strategies
• Maintain precise and timely documentation through HCHB
• Collaborate closely with interdisciplinary team members
• Participate in case conferences, staff meetings, and required ongoing training
• Uphold all applicable regulatory standards and agency policies

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
