Seeking an experienced Registered Nurse with a passion for home health to provide exceptional care to patients throughout a 13-week, full-time contract. This position offers the chance to make a difference in patients lives through skilled clinical care, accurate documentation, and effective scheduling.
Schedule:
- Full-time, Monday through Friday
- On-call availability may be required
Key Responsibilities:
- Deliver high-quality in-home nursing care and skilled interventions
- Complete patient assessments and timely, accurate OASIS documentation
- Collaborate with physicians and interdisciplinary care teams to implement personalized care plans
- Perform wound care, medication management, patient education, and ongoing disease monitoring
- Communicate changes or updates in patient condition to the clinical team
Desired Qualifications:
- Active RN license
- Minimum one year of recent home health RN experience
- Proven experience with OASIS documentation
- Familiarity with HomeCare HomeBase (highly preferred)
- Able to manage a structured, full-time caseload
- Reliable vehicle and valid drivers license
