Schedule/Hours:

  • No mandatory weekend rotations unless interested
  • 75% holiday work commitment
  • 13-week schedule cycles
  • Manager-approved time off (RTO)

Qualifications:

  • Current RN license
  • Minimum 1 year home health experience
  • Homecare Homebase (HCHB) expertiserequired
  • BLS certification
  • Valid drivers license and proof of insurance
  • Proven skills in IV administration, pleurx and wound vac use, wound care, comprehensive assessment, disease education, OASIS, and episode management (overseeing 2530 patients)

Responsibilities:

  • Independently perform at least six patient visits per dayroutine, discharge, ROC, recert, reassessment
  • Manage a caseload throughout Albemarle, Badin, Harrisburg, Mt Gilead, Norwood, Troy, and surrounding cities
  • Schedule visits directly with patients
  • Utilize technology tools (Samsung Note 9, EPIC Rover view, Focura)
  • Document care and manage patient data using HCHB
  • Collaborate with departmental leadership, schedulers, and clinical teams
  • Wear navy blue scrubs
