Schedule/Hours:
- No mandatory weekend rotations unless interested
- 75% holiday work commitment
- 13-week schedule cycles
- Manager-approved time off (RTO)
Qualifications:
- Current RN license
- Minimum 1 year home health experience
- Homecare Homebase (HCHB) expertiserequired
- BLS certification
- Valid drivers license and proof of insurance
- Proven skills in IV administration, pleurx and wound vac use, wound care, comprehensive assessment, disease education, OASIS, and episode management (overseeing 2530 patients)
Responsibilities:
- Independently perform at least six patient visits per dayroutine, discharge, ROC, recert, reassessment
- Manage a caseload throughout Albemarle, Badin, Harrisburg, Mt Gilead, Norwood, Troy, and surrounding cities
- Schedule visits directly with patients
- Utilize technology tools (Samsung Note 9, EPIC Rover view, Focura)
- Document care and manage patient data using HCHB
- Collaborate with departmental leadership, schedulers, and clinical teams
- Wear navy blue scrubs
