Step into a dynamic field-based nursing role providing dedicated care to patients in their homes. This full-time, 13-week contract offers the opportunity to make a meaningful impact through high-quality home health nursing while collaborating with a skilled clinical team.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Perform comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing care, including wound management, IV therapy, medication administration, and chronic disease monitoring.
• Monitor patient progress, detect changes in condition, and communicate promptly with healthcare providers.
• Educate patients and families regarding disease management, medications, and preventive strategies.
• Complete accurate and timely documentation within Home Care Home Base (HCHB).
• Collaborate effectively with multidisciplinary professionals to ensure coordinated, patient-centered care.
• Participate in staff meetings, case conferences, and required continuing education sessions.
• Adhere to all regulatory standards and documentation requirements.
• Travel daily within the assigned territory (mileage reimbursement provided), visiting 35 patients per day to meet weekly caseload expectations.

Qualifications:

• Current, active RN license (in good standing) for the state.
• Minimum 1 year of recent home health experience; field experience is highly preferred.
• Demonstrated proficiency using Home Care Home Base (HCHB) and conducting OASIS assessments.
• Reliable vehicle, valid drivers license, and current auto insurance are required.

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
