This full-time, 13-week contract role offers an impactful opportunity for a dedicated Registered Nurse seeking professional fulfillment and growth within the home health environment.

Schedule/Hours:

• Monday Friday, 8:00 AM 5:00 PM

Role Highlights:

• Daily travel within your assigned territory, with mileage reimbursement.
• Manage a caseload averaging 35 home visits per day (30 points per week).
• Utilize electronic documentation and OASIS assessment tools within Home Care Home Base (HCHB).

Qualifications:

• Active RN license in good standing (required in state of practice).
• Minimum one year of recent, field-based Home Health experience.
• Demonstrated proficiency with Home Care Home Base (HCHB) and OASIS tools.
• Reliable transportation, valid driver's license, and current auto insurance.

Key Responsibilities:

• Complete thorough OASIS assessments; design and adjust individualized patient care plans.
• Deliver skilled nursing caresuch as wound care, IV therapy, chronic disease management, and medication administration.
• Consistently monitor patient progress and proactively communicate updates or changes to the multidisciplinary care team.
• Provide patient and family education on diseases, medications, care plans, and preventive measures.
• Accurately document all visits and interventions in the HCHB system in compliance with agency and regulatory requirements.
• Collaborate with physical/occupational therapists, social workers, and home health aides. Attend conferences and participate in staff meetings and ongoing learning opportunities.

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
