Take your nursing experience into the field, supporting patients in their homes and making a meaningful impact on their day-to-day well-being. As part of a dynamic Home Health team, youll be responsible for delivering comprehensive patient care and collaborating closely with an interdisciplinary group of healthcare professionals.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract

Key Qualifications:

• Active RN license in the state of practice and in good standing
• At least 1 year of recent field-based Home Health nursing experience
• Proficient in using Home Care Home Base (HCHB) for electronic documentation
• Demonstrated experience completing OASIS assessments
• Reliable transportation, valid drivers license, and current auto insurance

Primary Responsibilities:

• Conduct OASIS assessments and develop individualized patient care plans
• Deliver skilled nursing care (including wound care, medication administration, IV therapy, and chronic disease management)
• Monitor patient progress and communicate any status changes to the healthcare team and physicians
• Educate patients and their families about diagnoses, treatments, and medications
• Complete accurate and timely documentation through HCHB
• Collaborate with physical therapists, occupational therapists, social workers, and home health aides to provide coordinated care
• Attend case conferences, staff meetings, and relevant continuing education as needed
• Ensure adherence to all regulatory and agency documentation and patient care standards
• Travel daily within your assigned territory to patient homes (mileage reimbursement provided)

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
