Your next nursing opportunity starts with a meaningful impact in home health. We're seeking a dedicated Registered Nurse (RN) with proven OASIS experience for a 13-week full-time contract. This position is ideal for nurses confident in clinical practice and thorough documentation, ready to support patients in their homes with compassion and professionalism.
Schedule:
- 13-week contract position with extension potential
- Full-time, Monday through Friday
- On-call coverage as needed
- Daily structured patient visit requirements
What You Will Do:
- Deliver skilled in-home nursing care, including wound care, medication management, patient education, and chronic disease monitoring
- Perform comprehensive patient assessments and complete OASIS documentation with attention to accuracy and timeliness
- Develop and implement individualized care plans in collaboration with physicians and interdisciplinary teams
- Maintain open communication regarding patient status and plan updates with the clinical team
- Ensure safe, effective care while adhering to regulatory standards
Qualifications:
- Active and unrestricted RN license
- At least 1 year of recent home health RN experience
- Demonstrated experience with OASIS documentation
- Proficiency with HomeCare HomeBase is highly preferred
- Reliable transportation and a valid drivers license
- Strong time management and organizational skills to manage daily visits
