What to Expect:
- 13-week assignment with the potential for extension
- Full-time hours, Monday through Friday
- On-call participation may be required
- Structured daily visit expectations ensure a consistent schedule
Key Responsibilities:
- Deliver skilled in-home nursing care, including wound care, medication administration, patient education, and chronic disease monitoring
- Perform comprehensive patient assessments and complete OASIS documentation accurately and promptly
- Collaborate closely with physicians and interdisciplinary care teams to develop and implement individualized care plans
- Communicate clearly and promptly regarding patient status and changes in condition
Who You Are:
- Current, active RN license
- At least 1 year of recent home health experience as an RN
- Proficient with OASIS assessments and documentation (required)
- HomeCare HomeBase proficiency is strongly preferred
- Confident managing a structured schedule of daily patient visits
- Possess reliable transportation and a valid drivers license

$2,193 - $2,312/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
