A rewarding role awaits an experienced Registered Nurse (RN) with strong OASIS expertise for a full-time, 13-week home health contract. This position offers the chance to make a meaningful difference in patients lives while advancing your clinical practice in a supportive environment.
Schedule:
- 13-week contract with potential for extension
- Full-time, Monday through Friday schedule
- Possible on-call responsibilities
- Structured daily visit expectations
Key Responsibilities:
- Deliver skilled in-home nursing care and clinical interventions
- Complete comprehensive patient assessments with timely, accurate OASIS documentation
- Collaborate with physicians and interdisciplinary care teams to develop individualized care plans
- Perform wound care, medication administration, disease monitoring, and patient/family education
- Maintain clear and thorough communication regarding patients status and care updates
Qualifications:
- Active RN license required
- Minimum one year of recent home health RN experience
- Strong proficiency with OASIS documentation (required)
- Experience with HomeCare HomeBase highly preferred
- Ability to independently manage a structured, full-time visit schedule
- Reliable transportation and a valid drivers license
