Your next nursing opportunity starts here. A dedicated Registered Nurse (RN) with OASIS experience is needed for a 13-week full-time contract in home health care. If you excel in clinical assessments, maintain thorough and timely documentation, and thrive in a fast-paced environment, this position will allow you to make a genuine impact on patient lives.
Schedule:
- 13-week contract, with potential to extend
- Full-time, Monday through Friday
- Some on-call responsibilities
- Daily structured patient visits
Key Responsibilities:
- Deliver skilled in-home nursing care, wound care, medication management, and comprehensive disease monitoring
- Conduct thorough patient assessments and complete OASIS documentation accurately and efficiently
- Develop and execute individualized care plans in collaboration with physicians and interdisciplinary care teams
- Educate patients and families on health maintenance and care strategies
- Communicate any updates or changes in patient status promptly to the clinical team
Qualifications:
- Active RN license in good standing
- A minimum of one years recent experience as a home health RN
- Proficient and confident with OASIS documentation
- Strong proficiency with HomeCare HomeBase (preferred)
- Ability to effectively manage a structured schedule and complete daily patient visits
- Reliable transportation and a valid drivers license
