Schedule:

• 13-week contract, with potential for extension
• Full-time, Monday through Friday schedule
• On-call coverage may be required

Key Responsibilities:

• Deliver comprehensive in-home nursing care and skilled interventions
• Complete patient assessments and submit OASIS documentation promptly and accurately
• Collaborate with physicians and interdisciplinary teams to create tailored care plans
• Provide wound care, medication administration, patient/caregiver education, and disease monitoring
• Communicate significant patient updates with the wider clinical team

Required Qualifications:

• Active, unrestricted RN license
• At least 1 year of recent home health RN experience
• Demonstrable OASIS documentation proficiency
• HomeCare HomeBase software experience strongly preferred
• Ability to manage structured schedules and daily visit volumes independently
• Reliable transportation and a valid drivers license
