Deliver essential care to patients in their homes as part of a dynamic clinical team committed to improving quality of life. This full-time, 13-week contract opportunity offers a Monday through Friday schedule from 8:00 AM to 5:00 PM, supporting individuals across the Renton, WA region. Daily caseloads average 35 patient visits, with weekly point targets and mileage reimbursement for travel within your assigned territory.

Key Responsibilities:

• Conduct thorough OASIS assessments, establishing tailored care plans for each patient.
• Deliver skilled nursing interventions such as wound management, medication administration, IV therapy, and chronic disease support.
• Monitor patient status, quickly identify changes in condition, and relay significant updates to physicians and the interdisciplinary team.
• Provide patient and family education on disease management, medication adherence, and preventive care.
• Complete timely and accurate documentation using Home Care Home Base (HCHB).
• Partner with therapists, social workers, and home health aides to coordinate and optimize care delivery.
• Attend case conferences, participate in team meetings, and engage in ongoing professional development.
• Comply with all applicable agency and regulatory documentation and care standards.

Desired Qualifications:

• Valid RN license in good standing in the state of practice.
• At least one year of recent home health nursing experience; field experience is highly preferred.
• Proficiency with Home Care Home Base (HCHB) and OASIS.
• Access to reliable transportation, current drivers license, and auto insurance.

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
