Your next nursing opportunity awaits! We are seeking a dedicated Registered Nurse with strong Home Health experience and expertise in OASIS documentation for a full-time, 13-week contract. This role is designed for a professional who thrives with independent patient care, excels in clinical documentation, and efficiently manages a structured schedule.
Schedule:
- 13-week contract, with potential for extension
- Full-time, Monday through Friday
- On-call duties may be required
Key Responsibilities:
- Deliver skilled nursing care to patients in their homes, tailored to individualized care plans
- Conduct comprehensive assessments and complete OASIS documentation promptly and accurately
- Collaborate closely with physicians and interdisciplinary care teams to coordinate patient care
- Provide wound care, oversee medication management, offer patient and caregiver education, and monitor disease progression
- Communicate changes in patient condition and care needs to clinical teams to ensure high-quality outcomes
Desired Qualifications:
- Active RN license
- At least 1 year of recent RN experience in home health
- Proficiency in OASIS assessment and documentation
- Experience with HomeCare HomeBase (strongly preferred)
- Strong organizational skills with the ability to manage daily visits reliably
- Valid drivers license and dependable transportation
