Bring your expertise and compassion to a dynamic home health environment, caring for patients in the comfort of their own homes. We are seeking a dedicated Registered Nurse with OASIS experience for a 13-week full-time contract. This role plays a pivotal part in helping individuals heal and thrive outside of the hospital setting, with ongoing potential for extension.
Schedule:
- Full-time, Monday through Friday
- On-call duties may be included
Key Responsibilities:
- Deliver skilled nursing care to home health patients, including assessments, medication management, wound care, and disease education
- Complete accurate and timely OASIS documentation for each visit
- Coordinate individualized care plans with physicians and interdisciplinary care teams
- Monitor and report patient progress or changes in condition to the clinical leadership
- Educate patients and families to empower safe, independent management of conditions
- Travel between patient locations with reliable transportation
Qualifications:
- Active and unrestricted RN license
- At least 1 year of recent experience as a home health RN
- Proficiency in OASIS assessment and documentation required
- Experience with HomeCare HomeBase (strongly preferred)
- Strong organizational skills to manage a structured schedule with daily patient visits
- Valid drivers license and dependable transportation
