Take on an impactful nursing position providing essential care in a home health setting. This full-time, 13-week contract involves delivering skilled nursing services to patients in their homes, ensuring they receive high-quality, compassionate attention outside the hospital environment.

Schedule & Setting:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based visits to patient homes within an assigned territory
• Average caseload: 30 points/week (typically 35 visits per day)

Qualifications:

• Active RN license (in good standing) in the state of practice
• At least 1 year of recent Home Health experience (field preferred)
• Proficiency with the Home Care Home Base (HCHB) electronic documentation system
• Strong experience completing OASIS assessments
• Reliable transportation, valid drivers license, and auto insurance

Primary Responsibilities:

• Conduct OASIS assessments and develop individualized care plans
• Deliver skilled nursing services, including wound care, medication administration, IV therapy, and chronic disease management
• Monitor patient progress and report changes promptly to the care team and physicians
• Educate patients and families about diagnoses, treatments, medication regimes, and preventive care
• Accurately document all care in the Home Care Home Base (HCHB) system
• Collaborate with an interdisciplinary team for coordinated patient care
• Participate in case conferences, staff meetings, and ongoing training
• Adhere to agency and regulatory standards for documentation and patient care delivery

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
