• Monday Friday, 8:00 AM 5:00 PM

Role Overview:

• Conduct OASIS assessments and develop individualized patient care plans.
• Deliver skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management.
• Evaluate patient status, communicate changes, and coordinate with physicians and other care team members.
• Educate patients and families on disease management and preventive care.
• Use Home Care Home Base (HCHB) for timely and accurate documentation.
• Collaborate with therapists, social workers, and home health aides for seamless care.
• Attend case conferences, staff meetings, and continuing education.
• Ensure documentation and care practices are compliant with regulations.

Ideal Background:

• Active RN license in Pennsylvania
• At least 1 year of recent home health (field) experience
• Proficient with Home Care Home Base (HCHB) and OASIS
• Valid drivers license, reliable transportation, and auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
