Job Description

Take on a vital nursing role delivering essential care to patients in their homes. This full-time, 13-week contract opportunity is ideal for experienced RNs passionate about supporting individuals in their communities with comprehensive and coordinated healthcare services.

Schedule & Setting:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based: daily travel to patient homes within an assigned territory (mileage reimbursement provided)
• Caseload: 30 points per week (typically 35 patient visits per day)

Primary Responsibilities:

• Conduct detailed OASIS assessments and develop individualized patient care plans.
• Deliver skilled nursing interventions, including wound care, medication administration, IV therapy, and chronic disease management.
• Perform ongoing evaluations, monitoring patient status, and communicate critical changes to the interdisciplinary care team and physicians.
• Educate patients and caregivers on health management, medications, and prevention strategies.
• Accurately document all care activities using Home Care Home Base (HCHB) in compliance with regulatory and agency standards.
• Collaborate with therapists, social workers, and aides to coordinate and optimize patient outcomes.
• Participate in case conferences and staff meetings, and pursue continuing education as needed.

Qualifications:

• Active RN license in good standing for the state of practice
• At least 1 year of recent field-based Home Health nursing experience
• Proficiency with Home Care Home Base (HCHB) and OASIS assessments
• Valid drivers license, reliable transportation, and appropriate auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
