This 13-week full-time contract places you at the forefront of patient-centered care, delivering essential services in a home-based setting. Schedule:
- Full-time, Monday through Friday
- On-call responsibilities may be required
- Structured daily visit schedule

Key Responsibilities:
- Deliver skilled in-home nursing care and interventions for assigned patients
- Perform thorough patient assessments and complete OASIS documentation accurately and promptly
- Work closely with physicians to design and implement individualized care plans
- Administer wound care, manage medications, provide patient and family education, and monitor chronic diseases
- Communicate observations and patient status changes to the clinical team in a timely manner
Qualifications:
- Active RN license
- At least 1 year of recent RN home health experience required
- Previous use of OASIS in clinical settings
- Proficiency with HomeCare HomeBase is strongly preferred
- Strong organizational and time management skills for maintaining a full-time, structured schedule
- Reliable transportation and valid drivers license
