Your next nursing opportunity starts here. We're seeking an experienced Registered Nurse (RN) to deliver high-quality in-home care for a full-time, 13-week home health contract. If you excel with OASIS documentation and thrive managing a set daily schedule, this role is ideal for you.

Schedule:

  • Full-time, Monday through Friday
  • Daily visit expectations
  • On-call responsibilities likely

Key Responsibilities:

  • Deliver skilled nursing care, interventions, and patient education in home settings
  • Complete patient assessments and OASIS documentation accurately and promptly
  • Collaborate with physicians and interdisciplinary teams to customize care plans
  • Conduct wound care, medication management, and monitor patient health status
  • Communicate significant developments to clinical team members

Required Qualifications:

  • Active RN license
  • Minimum 1 year of recent home health RN experience
  • Proficiency with OASIS documentation is essential
  • Experience with HomeCare HomeBase strongly preferred
  • Ability to consistently manage a structured, full-time visit schedule
  • Valid drivers license with reliable transportation
