Deliver quality patient-centered care as a registered nurse specializing in home health during a 13-week full-time assignment. Focused on field-based patient care within a supportive environment, this role allows you to impact patients lives while working independently and managing your schedule.

Schedule/Hours:

• Monday Friday, 8:00 AM 5:00 PM
• 13-week contract
• Expect 30 points per week (typically 35 patient visits daily)

Your Qualifications:

• Active RN license, in good standing, for the state of practice
• Minimum one year of recent home health nursing experience (field experience preferred)
• Strong proficiency with Home Care Home Base (HCHB) electronic documentation
• Thorough knowledge of conducting OASIS assessments (required)
• Valid drivers license, reliable transportation, and current auto insurance

What Youll Do:

• Perform comprehensive OASIS assessments and develop tailored care plans for each patient
• Deliver skilled nursing services such as wound care, IV therapy, medication administration, and chronic disease management in patients homes
• Monitor patient conditions, document changes, and immediately communicate with healthcare providers and the interdisciplinary team
• Thoroughly document care and visits in HCHB to maintain regulatory compliance
• Educate individuals and their families about disease management, therapies, medication protocols, and preventive care
• Collaborate with physical and occupational therapists, medical social workers, and home health aides
• Participate in case conferences, continuous education, and staff meetings to support best practices
• Travel daily within your assigned territory with provided mileage reimbursement

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
