Schedule:

Qualifications:
- Active RN license
- Minimum 1 year of recent home health RN experience
- Required experience with OASIS documentation
- Proficiency with HomeCare HomeBase strongly preferred
- Reliable transportation and valid driver's license
Responsibilities:
- Deliver high-quality in-home nursing care, including skilled interventions and patient assessments
- Complete OASIS documentation accurately and on time for each visit
- Perform wound care, medication management, patient education, and disease monitoring as needed
- Collaborate closely with physicians and multidisciplinary care teams to implement individualized care plans
- Effectively communicate any changes in patient condition to the clinical team for prompt follow-up

$2,193 - $2,312/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
