Your next nursing opportunity starts with a rewarding home health assignment supporting patients in their community. This full-time, 13-week contract position is perfect for a Registered Nurse (RN) who excels in autonomous work, precise documentation, and compassionate care. A strong understanding of OASIS documentation is essential in this role.
Schedule:
- Full-time, Monday through Friday
- On-call duties likely included
- Daily visit goals structured for consistency
Responsibilities include:
- Deliver skilled nursing care and interventions in patient homes
- Complete thorough assessments and timely, accurate OASIS documentation
- Develop and implement individualized care plans through collaboration with physicians and care teams
- Manage wound care, medication administration, patient/family education, and monitor health status
- Communicate changes in patient conditions effectively to the interdisciplinary team
Qualifications:
- Current and active RN license
- Minimum of one year of recent home health RN experience
- Demonstrated OASIS experience is required
- Proficiency with HomeCare HomeBase is strongly preferred
- Capable of independently managing a full-time schedule with consistent patient visits
- Reliable transportation and a valid drivers license
