A skilled RN with experience in certified home care and case management is needed to provide holistic, compassionate services across the lifespanfrom newborns to centenarians. This contract opportunity empowers you to work autonomously while engaging closely with both patients and their families.

Schedule/Hours:

• Must be open to weekend, holiday, and on-call rotations based on departmental needs
• Weekend rotation required once every six weeks

Qualifications & Skills:

• Active and unrestricted RN license in Massachusetts
• At least 1 year of certified home care experience
• Minimum of 1 year of case management background
• Reliable, independent transportation required for home visits
• Comfortable caring for patients of all ages, from birth through older adulthood
• Strong assessment, care plan development, and patient education skills
• Proficient in timely electronic documentation and interdisciplinary communication

Key Responsibilities:

• Provide skilled nursing care to a diverse patient population in their homes
• Assess, plan, and implement individualized care strategies
• Educate patients and families to promote optimal health, safety, and independence
• Complete admissions and fulfill case management duties in alignment with clinical policies and best practices
• Coordinate with interdisciplinary team members to ensure continuity and quality of care
