Qualifications / Desired Experience
- Current Registered Nurse licensure in Massachusetts
- Minimum 1 year of Certified Home Care experience
- Minimum 1 year of Case Management experience
- Reliable, independent transportation for home visits
- Willingness to work weekends, holidays, and participate in on-call rotations (weekend rotation once every 6 weeks)
Responsibilities / Duties
- Deliver clinical services to patients ranging from newborns to those over 100 years old in their homes
- Conduct thorough patient assessments and develop appropriate care plans
- Provide ongoing education and support to patients and their families to promote wellness and recovery
- Manage admissions and case management processes in compliance with organizational policies and standards
- Ensure accurate, timely electronic documentation and effective interdisciplinary communication
